How Many People Has BMI on Health Reports Actually Fooled?

The BMI index on health reports is actually a simple formula from over a hundred years ago. Its biggest blind spot is that it can't distinguish between muscle and fat weight. A muscular fitness coach might be labeled 'overweight,' while a 'thin' person with excess body fat shows as normal. But today I have to tell you the truth: this number isn't that sacred. It deceives people, and not just one or two.

First, let's talk about what BMI is. This thing was invented by a Belgian mathematician two hundred years ago. The formula is simple: weight (kg) divided by height (m) squared. Then it draws a line: below 18.5 is underweight, 18.5 to 24 is normal, 24 to 28 is overweight, and above 28 is obese. Sounds scientific, right?

But the problem lies in that 'simplicity.' It only cares about how much you weigh, not what that weight is made of.

Let me give you two real-life examples.

I have a buddy who's a fitness fanatic. He's all muscle, his chest could crush mosquitoes, and his body fat percentage is under 12%. He's 1.78 meters tall and weighs 88 kilograms. Calculate his BMI: 27.8, clearly 'overweight.' You tell him to lose weight? What would he lose? After losing that muscle, he wouldn't even recognize himself. On his health report, blood pressure, blood lipids, and blood sugar are all textbook perfect. BMI says he's 'unhealthy' – isn't that unfair?

Now look at the receptionist at my company. She's 1.6 meters tall and weighs less than 45 kilograms. Her BMI is only 17.5, which is 'underweight.' But she has takeout and milk tea every day, her body fat percentage can reach 28%, and the flesh on her arms is as soft as cotton candy. She gets out of breath after walking two steps. Her BMI looks good, but she has excess body fat, and her visceral fat might be higher than mine. You tell her to eat more? Everything she eats turns into sugar and oil, and what she gains is unhealthy fat.

See, that's the biggest pitfall of BMI – it can't distinguish between good meat and bad meat. It treats muscle and fat equally, counting them all as 'weight.' But we all know muscle is precious, and fat is harmful. A 'fat person' made of muscle and a 'thin person' made of fat might have the same BMI, but their health conditions are worlds apart.

There's more. BMI is especially unfriendly to middle-aged and elderly people. As people age, muscle naturally decreases, bone density drops, and weight goes down. Many elderly people have a BMI in the normal range, or even underweight, but they actually have sarcopenia – they're frail and can break a bone from a fall. At this point, BMI showing 'normal' actually masks the most dangerous signal. Similarly, some elderly people have big bellies and thin limbs, with BMI right at the normal line, but their visceral fat is already coating their liver and intestines, making them more dangerous than those with a high BMI.

So, BMI is fine as a reference. But treating it as a 'get-out-of-jail-free card' or a 'verdict' is just silly.

So what should we trust? Remember three words: look at body fat. Body fat percentage is the real hard indicator of how much fat you have. Normal adult men have a body fat percentage of 15%-18%, and women around 20%-25%. It's a hundred times more reliable than BMI. If you can't measure body fat, look at waist circumference. Men should keep it under 85 cm, women under 80 cm. Is the fat on your belly hard or soft? Hard fat is visceral fat, which is more dangerous.

So next time someone brings up BMI, just smile. Don't let a formula from two hundred years ago define your body now. Your health is up to you – we need to learn to look at the indicators that really matter.
